Traditional Therapy for acne includes oral antibiotics, topical treatments and for more severe acne, Accutane (isotretinoin). Many patients require “long-term” oral antibiotics for control of their inflammatory acne. Although generally considered safe, this has raised some concerns with patients and parents. Newer procedures and devices have been developed to help acne patients.

SmoothBeam Laser:
This laser system is FDA approved for acne and wrinkle treatment. For acne treatment it shrinks the oil glands (sebaceous gland), which can significantly improve acne and reduce the number of outbreaks. Also, this laser helps to rebuild collagen, which improves fine lines as well as acne scars.

V-Beam Laser:
This laser system is also FDA approved for treatment of vessels, red birthmarks, fine lines and other indications. For acne treatment, it will significantly diminish the “red spots” left by acne bumps. Also, it likely kills some of the acne causing bacteria on the skin by thermal (heat) mechanisms. This system not only improves the appearance of current acne, but also helps prevent acne as does the SmoothBeam Laser.

Microdermabrasion:
This is an extreme exfoliation of the skin. This system uses aluminum oxide crystals that are propelled against the skin to remove the outer layer of dead skin cells. This improves acne by helping to unplug many of the micro-comedones (black-heads and white-heads). It creates a smoother skin surface and helps the topical medications penetrate the skin more effectively.

Chemical Peels:
Mild salicylic acid chemical peels can help acne by exfoliation the skin as well. This helps unplug many of the pores to prevent bacteria from being trapped and causing an inflammatory reaction.

Extractions (acne surgery):
Although not true “surgery”, manually expressing the contents of black-heads and white-heads can help those acne areas improve quicker.

Blu-U Blue light treatment:
This FDA approved treatment is very simple and safe. Acne causing bacteria on your skin can be killed by using this light treatment. Bacteria produce by-products that are activated by blue light and these by-products are then toxic to the bacteria. This kills bacteria and can reduce or eliminate the need for oral antibiotics. It often works when oral antibiotics have not been effective.

Levulan with Blu-U:
Levulan in an aminolevulanic acid that is absorbed into the sebaceous glands and when activated with the blue light kills offending bacteria and destroys or shuts down oil glands delivering a one two punch to acne. You must stay out of the sun for 48 hours after this treatment.

What is the Candela GentleLase laser and why is it a safe treatment?

What does the treatment include?

Depending on the amount of hair treated, the procedure varies from patient to patient. Generally, the treatment will include:
Wearing safety eyewear to protect the practitioner and patient’s eyes from the laser light during the procedure.
A small hand piece will be used that touches the skin to deliver the laser light. A light spray of coolant onto the skin will be felt just before each laser pulse. Most patients experience little discomfort at the treatment site because of the dynamic cooling device.
The degree of discomfort varies with each person, depending on his/her pain threshold. Topical anesthesia may also be requested based on individual preferences or tolerances.

What precautions should be taken before and after treatment?

Avoid the sun 4-6 weeks before and after treatment or until your practitioner allows it. You must avoid electrolysis, plucking or waxing hair for 2 – 6 weeks prior to treatment.
If you have had a history of peri-oral herpes, prophylactic antiviral therapy may be prescribed.

How many treatments will I need?

Hair grows in cycles. The number of treatments required depends upon your skin color and coarseness of the hair. Laser hair removal requires multiple treatments.
Although one laser session can produce long-term hair removal, as a rule multiple treatment sessions are necessary to obtain optimal results. In large part this is due to the fact that laser hair removal is most effective for hair which is in the anagen phase.
Anagen is the growth phase of hair. Since hair grows in cycles, not all of the hairs are in the anagen at any given time. Additional sessions are necessary to catch all of the hairs when they are in anagen.

What will I look for after treatment?

Within several minutes of the treatment, the treated area will become slightly red and puffy. This reaction subsides, and within a few hours the area returns to normal. Since the laser does not cut the skin in any way, no bandages are necessary, and you can return to your normal activities immediately.
Patients will experience some discoloration or reddening of the skin around the treatment site. Some patients may experience hypo- (reduced pigment) or hyperpigmentation (increased pigment) response or increase of pigment color.
The skin will typically return to normal pigmentation over time.

How does the laser treatment work?
The laser creates a beam of high-intensity light that penetrates deeply into skin tissue, where it delivers a controlled amount of targeted therapeutic heat. Candela's patented dynamic cooling device (DCD) technology protects the upper layers of your skin with a cooling burst of cryogen. Together, the long-pulse laser and DCD cooling offer optimal treatment with minimal side effects.

How do I prepare for treatment?
If you have had a history of perioral herpes, prophylactic antiviral therapy may be prescribed. Your skin should be thoroughly cleansed and dried! Removing any makeup, creams, oils, topical anesthetics or self-tanning products before laser treatment. The intended treatment area should not be exposed to sunlight or tanning booth light for 14 days prior to treatment.

What can I expect during treatment?
Treatment will take place in your practitioner's office. You will wear safety eyewear to protect your eyes from the laser light. Your practitioner will guide the laser to the treatment areas with a small handpiece. Each laser pulse may be preceded and followed by a short pulse of cooling spray. A treatment could take as little as a few minutes to as long as an hour, depending on the area and condition being treated.

What can I expect after treatment?
A cool compress or gel pack may be applied for comfort following your treatment. Any redness or swelling should resolve within a few hours.

Does it hurt?
Some patients may feel a slight discomfort
With each laser pulse, sometimes described as the snapping of a rubber band on the skin. This discomfort resolves in several seconds and is minimized by candela's dcd cooling. For some patients, a topical anesthetic may be used prior to treatment. Be sure to discuss this with your practitioner.

How many treatment sessions will I need?
The number of sessions depends on the treatment you require and the condition of your skin. Many factors impact the efficacy of individual laser treatments, including the severity of the condition being treated.

What precautions should I take after treatment?
After laser treatment, prevent sun exposure by applying sunblock (spf 30 or above). In the first few days after treatment, don't scrub or rinse the treated areas with abrasive skin cleansers. Your practitioner may advise additional precautions.

Are there side effects?
Your risks are minimal. Side effects may include redness, swelling, hyperpigmentation, hypopigmentation, blistering and purpura (a laser bruise) in the treated area-these usually do not last more than a few days. Permanent side effects are rare. Be sure to discuss potential side effects with your practitioner.

Are these treatments covered by insurance?
Most insurance companies do not offer reimbursement for cosmetic procedures. Consult your insurance carrier for conclusive information.

Candela VBeam Laser
VBeam Laser is effective in treating: Rosacea, Facial Veins, Leg Veins, Red Birth Marks, Angiomas and Poikiloderma (uneven coloring of the neck).
This laser delivers an intense but gentle burst of yellow light to the skin. The light is specifically absorbed by the blood vessels in the dermis. These blood vessels are coagulated and then reabsorbed by the body during the natural healing process. Usually more than one treatment is needed.
The epidermis is protected during the treatment by a dynamic cooling method, DCD, which sprays a cooling mist onto the skin before each laser pulse, increasing the patient's comfort.
Side effects may include purpura (a laser bruise), redness and swelling in the treated area. These are transient and usually do not last more than a few days. Permanent side effects such as scarring are rare.
It is recommended to avoid sun exposure before and throughout the course of the treatment. A sunblock (SPF 30 or above) should be used. The post-treatment care needed depends on what lesion was treated.
